Managing Multimorbidity, CKD and Frailty  

By the end of this session, delegates will be able to: 

  • Recognise and assess frailty in a primary care setting 
  • Balance medicines optimisation with the risks of over-treatment in people with multimorbidity 
  • Align medicines decisions with patient goals, quality of life and realistic outcomes 
  • Assess CKD risk and diagnosis confidently and review medications and monitoring accordingly  
  • Explain when maintaining current therapy or making no changes is clinically appropriate 

Deprescribing in CVD, CNS & Frailty with Confidence

  • Apply structured deprescribing approaches within SMRs 
  • Identify opportunities for deprescribing cardiovascular, CNS and frailty-related medicines 
  • Communicate risk and benefit clearly and confidently with patients 
  • Navigate patient and clinician anxiety around deprescribing decisions 
  • Explain when maintaining current therapy or making no changes is clinically appropriate

Dealing with Difficult ConversationsDependence-Forming Medicines & Mental Health 

  • Prepare for and structure conversations about mental health and dependence-forming medications confidently  
  • Use language that de-escalates conflict and supports shared decision-making 
  • Apply trauma-informed principles when discussing long-term dependence and change, and applying this to mental health reviews and consultations  
  • Spotting signs for signposting, referral and safety netting and counselling with confidence  

Interactive Consultation Skills WorkshopMental Health, Dependence-Forming Medications and Pain 

  • Apply a structured consultation framework to challenging SMR scenarios 
  • Learn to hold space for difficult conversations, such as mental health discussions, and navigate them confidently 
  • Respond to resistance, distress and uncertainty using patient-centred communication techniques, particularly in deprescribing dependence-forming medications and medications for pain 
  • Practise and reflect on consultation skills in a supportive, facilitated environment
