The Personalised Care Group invite you to attend a ‘Carers, Young Carers and UPC’ webinar. The aim of this webinar is to provide an overview of the information and guidance available on the Personalised Care Collaborative Platform to support the delivery of personalised care, across all components, for carers. It will include an interactive session to explore delivery support required and showcase examples of how good personalised care is being delivered for carers across the country.
This webinar is aimed at anyone who would like to know more about how personalised care might work for carers including commissioning managers, providers, carers leads, clinical staff, personalisation leads, social prescribing link workers, health coaches, personalised care co-ordinators, VCSE organisations and provider trusts.
