More people are living with and beyond cancer than ever before.
Research shows there is often a ‘post-treatment’ gap, where patients report feeling abandoned, with unmet psychological, physical and emotional support needs.
Bridges Self-Management increases the confidence, skills and knowledge of practitioners to provide personalised self-management support for people living with Cancer.
This innovative project will integrate the Bridges approach to supporting self- management within Cancer services and evaluate the impact on staff, patients, teams and service pathways. It has been fully funded by NHS England: East of England.
The project provides fully funded training in evidence-based personalised self- management support, for patient-facing staff working across the Cancer services in the East of England.
The training aims to build practitioners’:
- Knowledge of Bridges core principles of personalised self-management support and Bridges’ underlying evidence base that supports its effectiveness
- Skills and tools to embed the core principles throughout your clinical practice
- Confidence in working in this way with patients and in teams
The training is 4 x 2 hour interactive virtual workshops, spread over 6 weeks to allow time for implementation, confidence building and reflection. All teams also benefit from free ongoing support from Bridges, including coaching, networking and further training, to ensure changes are embedded and sustained.
