Topic: End of Life / Palliative Care
These sessions are for new and established care co-ordinators who are working in the NHS. We hold monthly share and learn sessions, co-chaired by the national Supported Self-Management (SSM) team and national and regional colleagues.
Purpose
These sessions have been developed to support you in your role. They add to what you already know about SSM and care co-ordination, including how to embed personalised care in your PCN by building a community of people who are passionate about improving people’s knowledge, confidence and skills to take more control of their health and wellbeing.
