- Showcase PCNs that have started to use data to design new models of proactive social prescribing to deliver improvements in health and wellbeing
- Exemplars of collaborative work with communities and the voluntary sector to support people with the most complex needs
- Learn how health and care partners are working in new ways to take collective responsibility to improve access to services, meet non-clinical demand and deliver better patient outcomes
