Module: Introductions- What is good link work?
What will module cover: Introduction into your role as a social prescribing link worker. Develop a better understanding of your role and what you can and cannot do for your patients. How to establish boundaries and acquire greater awareness of what good intervention looks like to provide the best support for your patients. The sessions learnings will be provided by Sian Brand who will pass on her insights as the Social Prescribing Associate. Link workers will also get an introduction to Care Network Cambridgeshire, what we do as a charity and how we can work together to provide the best support for people in the community.

Module: Introduction to the Voluntary Sector

What will module cover: An overview to what the voluntary sector has to offer. Link workers will have a better understanding of the voluntary sector, what different services provide and how you can match patients to the best services available. Once we have covered the bigger services, link workers will get a chance to expand their knowledge of what’s available in their local area through the breakout area sessions with the community navigators from their local district . Applying this knowledge to practice, link workers will then get a chance to pose questions to the navigators on possible solutions available to support their patients.
Day Two – 02/02/2022

Module: Introduction to Housing and Hoarding

Housing
- Who owns and manages homes?
- Different types of housing available.
- Introduction to the different housing partners and agencies across Cambridgeshire and Peterborough
- An understanding of how to support people get out of hospital and into suitable housing
- Homelessness –how the homelessness process works –
- People have multiple disadvantage e.g. mental health, physical health, drugs, alcohol, low income, poor housing
- Building affordable homes to prevent homelessness.
- Helping people improve their financial abilities and get back into work – various projects
- Hoarding
Hoarding
- What is a hoarding disorder?
- How to identify a hoarding disorder in a patient.
- How to support patients who have a hoarding disorder.
- Support available for you and your patient.

Module: Changing the Conversation and Strengths Based Practice

Module: Applying your learnings: Scenarios and How to Write Compelling Case Studies

What will module cover: This is an opportunity to meet with Joanna, who is also a social prescribing link worker. You can ask her any questions and find out tips on how to embed your service into the practice.
