In collaboration with regional Primary Care colleagues in the East of England region, we are pleased to offer a bitesize ‘lunch and learn’ webinar on Tuesday 8 March, 13:30-14:00.
This session is part of a series of webinars aimed at supporting PCNs to maximise the benefits of the three personalised care roles in primary care (social prescribing link worker, health and wellbeing coach, and care co-ordinator). It will run for thirty minutes (20 mins short presentation and 10 mins discussion). It will also be recorded for those who are unable to join.
The webinar will be facilitated by our regional Social Prescribing Facilitators and PCN Advisors, Tim Anfilogoff and Sian Brand.
Shraddha Puttaswamy, Regional Mentor for Care Coordination, will discuss “Care Co-ordinators in Primary Care Networks: Embedding the role, building networks with key stakeholders, developing referrals, and identifying training needs.”
