About this webinar
This webinar will support colleagues in primary care to understand how to implement personalised proactive care, using a targeted approach to reduce reliance on clinical input through improved biopsychosocial support. It will explain how to make best use of the skills and expertise of social prescribing link workers, health and wellbeing coaches and care co-ordinators to tackle population health needs. It will include examples from London and Dorset where primary care networks that are using targeted approaches to increase people’s ability to self manage and address their wider social and economic needs, reducing reliance on GP appointments.
The webinar will also highlight the proactive social prescribing service set out in the network contact DES, and the ongoing recruitment of social prescribing link workers, health and wellbeing coaches and care co-ordinators through the ARRS.
