We hope you will join us for a dissemination event on the findings of our Marie Curie funded SNAP2 Study – piloting the Support Needs Approach for Patients (SNAP).
SNAP is a 5-stage intervention to enable person-centred care in progressive disease: https://thesnap.org.uk/. It has been piloted with primary care, community care and hospital teams. We will introduce you to SNAP, share the views of patients and health care professionals who have used SNAP, and tell you how get a licence (free to not-for-profit organisations) to use SNAP.
Lunch is provided – please book your place to help us plan catering – spaces are strictly limited.
Parking: please use Babraham Park & Ride or Addenbrooke’s Hospital car parks (fee applies) – no on-site parking.
