Stort Valley and Villages PCN has created a Social Prescribing Service for 11–25-year-olds in support of young people’s health. By connecting young people to the right services, improving wellbeing, and preventing future problems, the PCN is changing lives and making a positive impact in the community, since its launch in 2019.
