In recognition of mental health week we have gathered a variety of speakers together to share their experiences of social prescribing and connecting with community assets to find ways to bridge the gap between clinical parts of the system and community based support, that supports the wider determinants of health, reduces loneliness and increases connection.

Social prescribing and the role of link workers in facilitating connection to assets in communities.
How social prescribing link workers are working proactively with people and communities to promote connection and reduce loneliness.
