The East of England Personalised Care team are offering a series of monthly PCN lunch and learn webinars. These are designed to provide quick bites of information around social prescribing and the three personalised care roles in primary care (social prescribing link worker, health and wellbeing coach, care-coordinator).
The webinars are open to all colleagues who lead on social prescribing and embedding the three personalised care roles in PCNs across the East of England region.
