Health & Wellbeing Coaches for PCNs. What you need to know to make the most of this ARRS role and opportunity in delivering Personalised Care for your patients

The PCN Webinars are a monthly series (30 minutes long each) that are being developed by Sian Brand and Tim Anfilogoff as a part the East of England Regional PCN Advice support.
The webinars will run initially until March 2022. This offer is led nationally by the Social Prescribing Team and the Personalised Care Group but supported by the Regional NHS team by Hazel Grace with lead Susanne Cox.

The purpose of the webinars is to give pertinent information about the three personalised care ARRS roles in PCNs – the Social Prescribing Link Worker, The Health and Wellbeing Coach and the Care Co-ordinator.

The focus is around developing high quality safe practice in these new operating models for primary care, offering direct PCN support as well, where needed. The webinars are also developed with the knowledge and support of Siobhan Taylor in the Regional Primary Care Team and will be posted on the regional NHSEI YouTube website afterwards.
