This is a fully funded online workshop for Carers, Healthcare Assistants and Support Workers who may be having advance care planning discussions working within a generalist setting, such as, the community, care homes and acute trusts.
During this session you will:
- Identify what personalised care and advance care planning means
- Explore aspects of future care wishes and how to discuss them
- Review documents commonly used in practise
- Gain awareness of specialist services and what they can offer.
