Who are these sessions for?
These sessions are for new and established care co-ordinators who are working in the NHS. We hold regular share and learn sessions for care co-ordinators focusing on relevant topics. They are chaired by the national Supported Self-Management (SSM) team.
Purpose
These sessions have been developed to support you in your role. They add to what you already know about SSM and care co-ordination, including how to embed personalised care in your PCN by building a community of people who are passionate about improving people’s knowledge, confidence and skills to take more control of their health and wellbeing.
