People with severe mental illness are at a much greater risk of poor physical health and die on average 15 to 20 years earlier than the general population. Although more annual physical health checks for people living with SMI have been delivered than ever before there’s still more to do. We need to be ensuring that not only are people getting these checks but they’re receiving support for follow-up interventions to look after their physical health, in a way that’s personalised for them.
The RightCare scenario compares a suboptimal – but realistic – patient journey with an optimal one, highlighting the key elements of optimal care across the patient pathway. Join this webinar to find out more about the scenario and what systems can do to improve the physical health of people with severe mental illness.
We are also joined by City and Hackney who will provide a spotlight on the work they have done through partnership working between primary care, secondary care and the voluntary, community and social enterprise (VCSE) sector to deliver SMI physical health checks and provide tailored support to improve the physical health of those living with SMI.
