The Community Diagnostic Centres will be located at North Cambs Hospital (Wisbech CDC) and Princess of Wales Hospital (Ely CDC). We will talk about the tests available and how you can access them. We are interested in supporting primary care to access rapid diagnostics and setting up proformas to facilitate enhanced pathways. We hope to have GP representation alongside our secondary care colleagues to develop some of these pathways such as breathlessness and liver disease. We also hope to adopt some of the pathways recently developed such as the optimal lung cancer pathway and the sarcoma pathway. Your input is pivotal to ensure we are meeting our patient needs.
CUH provides virtual ward service as part of the national initiative to enable more patients to receive hospital care at home. The service was opened in November 2022 and has looked after over 1,200 patients to date. The CUH virtual ward differs to other virtual ward services by ensuring that the patients remain under consultant lead care, they have 24/7 direct access to the virtual ward staff and the trust continues to take responsibility for care, medicines management, investigations and monitoring. Thus far we have had to concentrate on step down care as the region already has multiple admission avoidance teams but we are looking to improve community integration and develop direct virtual ward admission pathways.
