Virtual half-day Annual Conference – thanks to generous sponsorship and support from the Royal College of General Practitioners – will serve as the culmination of our second annual Personalised Care Week, a four-day ‘roadshow’ of webinars, videos and educational activities exploring current and relevant topics related to personalised care and multimorbidity.

Multimorbidity has an impact on those living with multiple long-term health conditions, their carers and the health system – with people often finding it challenging to navigate a range of services and treatment plans. Titled ‘The Multimorbidity Challenge: A Personalised Care Approach’, this year’s conference will explore how health and care professionals and services can utilise personalised care in multimorbidity to meet the changing needs of our population and improve people’s experience of illness.

It will feature a keynote address from Sir David Haslam – a former Chair of NICE, as well as the former President and Chair of the RCGP, and a notable writer and healthcare policy consultant.

As well as Sir David, the conference will include addresses from a variety of leading personalised care and multimorbidity experts – as well as people with lived experience of multiple long term health conditions – and will cover a range of important areas of work related to multimorbidity, including hospital admissions, ethnic inequalities and healthy ageing.
