Analysis of patient activity at a Blackpool GP medical centre in one of the most deprived wards in England uncovered that 3% of their patients of all age groups had high attendances across the health system and could potentially be better supported with an enhanced model of care.

In this session Dr John Jeans, National Lead for the CLEAR programme interviews Dr Neil Hartley Smith, GP and Clinical Director for Blackpool and Fylde and Wyre Clinical Commissioning Groups on the pressures primary care are facing, what the CLEAR data uncovered and how a new multidisciplinary team could better serve their needs while relieving pressure on the whole health and care system.
