Nurse educational forums are designed for Practice Nurses across Peterborough and Cambridgeshire and facilitated by the C&P Training Hub Clinical Nurse Educators. The objective of these nurse forums is to provide valuable learning opportunities with a guest speaker at each event. It also gives Practice Nurses, the opportunity to network and aid collaborate learning.
Guest Speaker – Morag Farquhar – There is a brilliant tool – the SNAP tool (Support Needs Approach for Patients) – to facilitate identification of patients’ needs for support. It is for patients with Long Term Conditions / progressive disease. It has been developed & validated by a team of nurses working for Cambridge University & University of East Anglia. (I have copied in Morag Farquhar who is one of the team and who would be delighted to talk at any or all of the Practice Nurse Forums if you are interested).
The tool acts as a prompt to patients / gives patients ‘permission’ to share their needs with the clinician. It is designed to be easy to use (& time efficient) for both the patient & the clinician and truly supports a patient-centred approach to assessment of need and joint problem solving between patient and clinician.
