Join us for a webinar on the topic of the proposed minimum data set for social prescribing. The minimum data set (MDS) has been developed in partnership with a number of link workers, GPs and other stakeholders across the country through an early adopter programme, and can be used within both GP IT systems and non-GP IT environments.

This webinar is aimed at social prescribing link workers and those with similar roles using digital case management systems either in GP practices or in other settings

Aims:

  • Learn how the minimum data set (MDS) can be included in digital social prescribing case management systems to record an individual’s journey and support high-quality social prescribing delivery
  • See in practice how the MDS can be used in:
    a) EMIS GP IT system
    b)  An in-house case management system in a non-GP environment
  • An opportunity to ask questions
  • Next Steps: Hear updates on
    –  progression of the Social Prescribing Information Standard
    –  development of a SNOMED reference set for social prescribing
    –  implications for use in software case management systems
