Health Coach Training – Funded Places Available

Personalised Care NHSE funding has been allocated to supporting Health Coach Training. These places are suitable for Social Prescribers and Health and Wellbeing Coaches working in Primary Care within C&P.

The training provider, Peak Health Coaching, offers 2 and 4 day training that has been accredited by the Personalised Care Institute.

Peak Health Coaching has developed its own tailored Health Coaching approach, based on many years of experience of patients’ personal success stories and follows an intuitive path towards behaviour change.

The Tailored Health Coaching Training encompasses a range of coaching models and approaches together with the concept of activation to ensure that participants leave with a practical approach they can use with their very next patient or client.

Shared Decision Making

Shared decision making (SDM) is a process by which people are supported to understand their options and are given the opportunity to consider relevant information that might influence their choice.

By 30 September 2022, a PCN must ensure all clinical staff complete the Personalised Care Institute’s 30-min e-learning refresher training for Shared Decision Making (SDM) conversations

This three-week course brought to you by the Leadership for Personalised Care team has been developed via the FutureLearn platform. It will give you an introduction to the meaning and importance of personalised care and equip you with the tools you need to be an effective leader.
